I completely agree with Patty, a board with these design rules should be a
no-brainer for any competent shop.  Susan, if you want to give your supplier
a temporary method of culling bad boards while they solve their process
problems, consider electrical test.  I've never done this on a double-sided
board, since the flaws are usually caught on visual inspection, but in this
case, it could buy your supplier a little bit of time to solve their
problem.  If they can't tell you what went wrong and how they fixed it
pretty soon, I'd change ships (or shops).
