Very simply, we would attach a seperate sheet along with the travellar for
the operators / inspectors to fill out any time there would be rework or
repair. This sheet would also be identified to the customer part number,
etc.

On this sheet would be different columns, one for repair, one for rework,
one for scrap, date, person etc.

In addition, the operators would fill in how many pieces beside the
appropriate column and would also enter in a code taken from a
predetermided list. These codes relate to the problem seen (mask on pads,
voids, low plating, etc.)

At the end the codes and quatities could be entered into any spreadsheet -
such as Excel, and a Pareto analysis can be run using macro commands.

This makes it very usefull to see which problems you need to attack first.
