Phil,

What design programs are you using?  We use Cadstar and what we've
done is incorporate a unique symbol on layers "top drill" and "bottom
drill".  When the board is finished I turn off everything except
those layers and the board outline.  Then I export it as a .dfx file.
 That gets imported into AutoCAD for the drill drawing.  Perhaps your
programs will allow you to do something similar.
