Eeek, that do look UGLY... I am going to guess.. the pits COULD be caused by particulate in the soldermask developer.   What would cause particulate in the soldermask developer is anyone's guess... maybe some dirt fell in...

Should be easy to prevent, if someone will simply inspect board as it comes out of the developer..
