PCB Pro <http://www.pcbpro.com/pcb-quote.php?nbrLayers=2> has an
automatic instant quote system that will give you an idea of what the
cost should be. Their pricing is good, I think, if you fit into what's
standard for them.
I had good results with ECS <http://www.ecspcb.com/>
(http://www.ecspcb.com/). With them You can go outside of what's
standard for a McFabhouse and the pricing is still comparable to PCB Pro
/ PCB Express. Once ECS absorbed for us part of the cost of an error
that resulted from data format confusion, which is something an
automated fab house probably would never do.
