This appears to be an area of metal that was not etched off in the fab
process - likely caused from damage to the photo resist before plating.
It also appears that the removal of the extraneous metal was attempted
as the leading edge has been lifted. Possibly was mixed back into the
lot accidentally when rework-ability issues encountered.

Hi Ofer:
It seems that an object smashed against the area affected. There
is a mark left by an object that collided with the area.

Ramon

Subject: [TN] Loose peace of metal - why?
Technettters,
My QC inspector called me to watch the phenomena showed in the picture
that Steve Gregory kindly loaded to his site, and can be found in
http://www.stevezeva.homestead.com[Ofer Cohen] (Look for "14 PCB
0402"). This is an extension to the edge trace, but is loose and seems
to be breakable.
Can somebody tell me why it happened and how to avoid it?
Kind regards,
Ofer Cohen
