Here is the materials declaration sheet we are using, (MEETS all NEMI
requirements). This is same as being used by large PC manufacturers (I
know of) and is be sufficient for the current WEEE requirements, feel
free to take and use as is or format for your own needs.

Not yet generally available - still in draft and available only to the
privileged few.
I have tried getting copies from CENELEC/IEC and a CENELEC committee
person at the BSI, all to no avail. The committee person said it was
available in draft from the BSI. The BSI customer services said it is
not available. I have copied the DTi on this matter.
As I said to the committee member, it is ridiculous that such
documentation is not generally available for comment especially to those
at the coal face of industry and who best placed to comment on the
practicalities.

All,
I have few questions in regards to Material Declaration forms.
(1) Is there a standard Material Declaration form?
(2) Where can I find such form?
(3) Do you consider CoC (Certificate of Compliance) and MD (Material
Declaration) forms to be different? I think both are different.
(4) Any suggestion on preferred method of storing them, I meant which
department should be in charge of those (in your eyes) so that we can
pull
out when required/asked by authority in European countries?
This will help me in chalking out my plan in assigning action items.
