Paul,

I can give you some ideas on the cause:

Pinholes in solder mask typically have a few possible sources, depending on the application and type of the solder mask.  Screen printed solder mask can have pinholes from plugged screens.  Photoimagable solder mask can get pin holes from bad artwork or debris on the artwork or exposure frame.
