A test coupon may not necessarily give you all the data...cross-section a
suspect board, the via that is failing. If hole wall copper thickness is
uniformly thin then I would suspect 1) poor electroless coverage 2) poor
electroplating coverage 3) over-etched. If you find adequate thickness with
the expection of low/thin spots (voids) then agitation in electroless or
even electrolytic may be the cause... I'm sure if you wrok close with the
fabricator the problem would be easily resolved...
