In IPC-2222 (i.e. the previous version) it's "Maximum hole to minimum lead diameter" and "Minimum hole to maximum lead diameter".  I don't have issue A in front of me here.

You may have found one of the deliberate mistakes the IPC included to keep people that look for mistakes happy :-)
