First off, get rid of that oversized entry you mentioned. Get slightly
undersized entry and tape on two sides only.

Some questions you need to ask:

What has your drill supplier done to help?

What type of material and how many layers are you drilling?

Is the material fully cured?

Is there sufficient vaccuum?

Is there sufficient pressure from the pressure feet?
