Custom Design and Photo, (203)265-3690 provides scanning services. They can
output the scanned data in gerber or IPC-D-350 formats. The majority of
requests have been for the gerber formats. Your CAD system must be able to
input the gerber format in order for you to enter it into your CAD data
base. Another option you have, is to read it into the ACT 350 (408) 358-4691
CAM package. They have the ability to reverse engineer to an intelligent
database. 

I'm sure that there are other software packages that are also used for this
purpose.
