Generally this is caused by inadequate exchange of solution in the anode
bag with bulk solution.  The weave of the anode bag is either too tight
or has become plugged.  Interior solution continues to build in CuSO4
concentration due to anode disolution until the interior solution
becomes super saturated and crystallization is inevitable.  Either clean
andode bags or reduce the nap and weave in future anode bags.
