Discussions with the pcb vendor revealed a potential problem with gloss white mask product the were using. They sampled us the same pcbs using another white gloss mask, which we ran using the same processes and reflow profile, and didn't have the problem. The pcb vendor has changed their white mask recipe as a result, and we have not seen it since.
