Guys, 

We are doing some CM and EMS jobs on the side as we have extra line time 
with our new lines. Seems like the people that are securing our jobs are not 
asking for the information/data that we we require or could be provided. We 
have different people and departments that need to get the jobs started and 
done without pulling the customers teeth a few times by few different people. 

Does anyone have a document/checklist of all the information that is needed 
or can be provided for a CM/EMS jobs that they have had to fill out for a CM 
or EMS. Don't want to reinvent the wheel here.
