Change IPC-6012 Designation to Type 5 or Type 6.

Add requirements for hole filling (material, etc) to create metal core
isolation.

Specify the IPC-6012 optional test "Circuit/Plated-Through Shorts to Metal
Substrate" test, ref IPC-6012B section 3.8.3
