I have no documentation on specific problems that may have caused the creation of this guideline. The failure mode is a cracked cap. Usually doesn't show up in ICT but fails short later (normal X7R, not open mode part). The board stress due to the shear wheel doesn't look like much to me. We had some X7R parts fail by shorting in the vicinity of a score line, but ended up attributing it to voltage stress, not physical stress. We say 7MM in our guidelines but can't always comply. Orientation is also a factor. Parallel is better than perpindicular (to the Vscore. Same caps from different sources can make a difference too.
