Hi Joe: It could be several things. Like you say, there may not be a way for the air to evacuate from the connector side because it is plugged up and the solder can not displace the air so it does not travel up the THT. Preheating the board and immediately sending them through the wave solder helps. In addition, touch up the leads that are connected to ground planes by hand using a high thermal transfer tip. Another, very unlikely cause, may be that you have solder ability problems with the barrels. The outer metal shells are olive drab iridite over cad plate, and they mate together with a twist/lock collar. When the suspect solder joint was touched with an iron and some Soder Wick, the whole thing disappeared. There appeared to be good wetting to the lead and the pad, but very little flow up into the barrel.
