Usually what I see are typos, label orientation, packets not placed in the
correct 'corner' and other stupid a** "errors" that can more easily be
corrected by a pen, razor blade, and 30 seconds of time...

The Department of Transportation regulates the shipment of hazardous
materials. They specify the correct labeling and packaging of these
materials, not the carrier. If as stated 25% are going out incorrectly
labeled or packaged it is just a matter of time before one is found by
the DOT in transportation. I have read numerous accounts of mislabeled
packages leaking and found by the DOT and the fines were not small
especially for air shipments.
