Gary, Had smilar problems with a cap of this same type of construction and they were manufactured by Sprague.... There was a problem with the seal that goes around the termination that sticks out of the conformal coating.... In 1 lot (+10K) the seal/insulator was not sealing properly and since it, and the electrode tip were exposed, they were subject to thermal-mechanical damage which degraded the seal further... This allowed material into and damaged the electrode insulation gap which caused the capacitor to short under load. We replaced the caps with a new "improved lot" and the problem went away... We then replaced all caps of this package design with the molded type to eliminate the possibility of future issues..... From our electrical engineers statement the most common failure is during in-circuit test. This is conducted at ambient temperature & humidity. These capacitors are used to decouple an input voltage, which is set to increasing values. The failures have been occurring when a steady-state input voltage of around 38 VDC is used. When the voltage is left on, the chances of failure increases.
