John, Normally that information is supplied on the PCB Fabrication drawing. I don't think it is necessary to ask the fabricator where they get their material from as long as it meets the requirements of the material that is specified on the Fabrication drawing. Sorry, I don't use a form. I select my materials and process by the type of electronics I am designing.
