I don't know about the insurance question.  As for the other, it is common to have the operators wear finger badges that show the total accumulated amount of exposure.  There are a number of companies that will set up a full program for you, including badges and analysis of the badges, normally quarterly.  Your plant Safety manager should be the point contact.  There are a couple of systems that specialize in low output tubes with very sensitive detectors.  The total x-ray exposure for any operator is so low that the systems are certified to run "open", that is with no cabinet.
