The Johnson and Grahm book, High-Speed Digital Design: A Handbook of Black Magic, is the best I know of. Mark Montrose also has a good book on the subject (sorry cant find the reference right now -- try http://www.amazon.com ) You can also check the PCB Design Confrence Proceeding for the last few years, the articles by Lee Ritchie and Rick Hartley are must reads for high speed designers.
