Steve,

My initial reaction to your comments are that the fabricator has liquid leaking back out of the vias after curing.

I'll explain why I believe this:
  a.. If it were a curing problem causing peeling it would be more widespread
  b.. If it were poor adherance at the copper (i.e. via pads, trace, etc) you would see peeling in those other places as well
  c.. If the copper was contaminated, it would also be affecting other areas than vias.
  d.. Component holes being larger than vias would drain of liquids better than vias
  e.. Vias being tented can entrap liquid if precautions and proper processing are not performed, and this is what I believe is happening.
I have seen this happen a few times here at our place.

Keep in mind I am basing this solely on the information available, I could be way off base...

Curious to know how this evolves though...
