Guy,

Check your connector, specifically the locks/clamps that hold the SIMM module in the
socket.  We had a problem where during shipment or if the board was flexed the SIMM
would loose contact with the connector.  Once the SIMM was re-seated the problem sent
away for xx months.  We changed the connector to a different vendor with a different
clamp/lock style that held the SIMM in mode securely (closer to 90 degrees vertical).
Send me an e-mail if you want the old/new connector vendor names.
