in circuitry manufacturing - volcano holes usually occur when the hole walls
are underplated. can get to be sooo bad that when u wave solder a board -
it'll suck the component thru.

if u r sure they are definite shaped volcano holes - what is happening is
that the hole wall cannot take anymore thermal shock - and so it blows. just
for the heck of it - have someone take a copper thickness reading on the thru
holes - if u get less than 0.80 mils - u have a very good chance of anything
underplated volcanoing on u.
