Nicholas -

The root cause of pin holes in the plated thru hole wall generally goes back
to the cleaning and activation of the hole or the electroless deposition
process.  Check these processes and chemistries very carefully.

As Gaby noted - the result is often an entrapment of chemicals which has an
adverse affect on solderability.  These entrapments also have a habit of
blowing bubbles during the wave soldering process, resulting in solder
eruptions and voids, for which there is no permanent cure, and the product
ends up in the scrap bin.
