If I were you , I would count it as 3 defects....The way we do it here is to
calculate the number of OPPORTUNITY OF ERRORS (OFE) and then we see how many
defects per million opportunities of error we make...

In other words , if I need to solder ONE SINGLE SMD resistor , I can make 4
errors...

*       1) I may use a wrong part
*       2) I don't place the part at all - MISSING
*       3) I place the part but it is defective on one lead
(misaligned/unsoldered etc)
*       4) I place the part but it is defective on the other lead
(misaligned/unsoldered etc)

The number of OFE is subjective and is usually given by the QA department .

If I count it as 1 , I call it "DEFECTIVE" ...if I count as 3 , i call it
"DEFECTS "

Kind of confusing , ain't it ?
