Folks,
in my experience, removing unused pads is definitely something that requires
customer's approval - there's just no question about it.
sometimes, though the removal of unused pads is not only recommended but
necessary: for example when (after etch factor) your spacing is less than 2
mils.  When the density of the PCB goes up, there may not be much of a
choice but to convince the customer to let you remove any non-functional
copper lands.
other than in cases like that, unused pads do not present a problem for the
board manufacturer and since they are part of the board's artwork, they
simply can not be removed without the approval of the owner of that artwork.
