I use Infinite Graphics in Minneapolis, MN  ph 612 721-6283 for process
verification plates.  I typically request black artwork on .090 thick glass
to an 1/8 mil tolerance, although you can go tighter.  They'll turn in less
than a week, and I have always been satisfied.
