Dave,
Here the answers to your questions, as I know them:

1  How do you notify your supplier of the requirement?  How do they
respond?  We have written the requirement into every component specification
(and let me tell you what a task that was!) and these of course are given to
anyone trying to supply us with particular components.
2  How do you screen your suppliers deliveries?  what rate do you sample
at?  Since this was done at the corporate level, I do not have an answer for
you on this holiday Monday, but I will get one.  My guess is not often
enough!
3  What kind of in-coming tests do you run?  Are the tested component
are "sarificed?"  same as #2
4  What kind of receiving history do have?  Are there suppliers /
components that do better than others? same as #2

Sorry for the lack of answers now, but I wanted people to know I am working
on getting answers to these questions.  We are trying to be a lot more open
in the hopes that others will respond in kind and that we can move the
industry and the standards forward together.  No company can really afford
to carry a complete set of its own standards any more and with the contract
manufacturing industry out there, it doesn't even make sense to try and do
it.  Long live the IPC!  Oops, some Britush Commonwealth influence coming
through.  :)
