I have a 1998 publications book here from the IPC.  It appears a whole set of SMT documents including the one mentioned costs $620 (US) for a nonmember.  There is a note on the bottom to visit http://www.ipc.org/html/pubsvideos.htm for means to purchase the item or 847.509.9700 ext 395
