Ed,

One more thought on this topic.  If this is a multilayer board than its
possible that you could have localized heat sinking of the PTH's to an
internal power or ground plane.   I have seen in the past (via
microsections) where the solder flows up the barrel and stops right at the
internal plane.

If this is the case then its your customers design and or wave parameters
that are at fault.
