Yes, I have had the problem also. One thing to check is the amount of plating
on your diodes. The last time I encountered this problem, I found that the
supplier that purchasing was using simply dipped the diode leads into a solder
pot, and the plating thickness varied wildly. By using a different manufacturer,
the problem went away for that run of pcb's. The older diodes were simply kept
on the shelf until the pcb's were recut and the hole made larger.
