   I  understand your problem. We tried many of the things you listed with little success. There are
several things that can be done. Your in-circuit test engineers can develop a test test requires a probe to
touch one end of the cap and will show a fail when installed wrong. Another is to always orient all the
caps in the same direction. This helps the hand assembly people get used to the same orientation. The best
method is to have a prep on one of the leads which doubles over the end of the lead, making it twice as
thick. Than lay out the board with one hole larger than the other. We found this to be 100% on Aluminum
Electrolytics. Hope this can help. Let me know if it does.
