In the absence of any other feedback, here are our guidlines: Notebooks must be bound
Date of commencement and when finished date of completion to be entered on the front of the book
All entries to be in ink
All entries to be dated
All entries to be legible
Spaces between entries to be kept to a minimum

If you think about the possible uses of a log book the rules become clear. i.e. you may be required to back up a decision /conversation/discussion at some point in the future,  your log book should contain any actions of note that you have carried out that may be contentious/likely to be questioned.
The log book is also YOUR place of reference, mine has saved my backside many times.
