Hi Hugh,
        I have the following suggestions for your survey.

1) Product Mix- include controlled impedance boards

2) Design Capabilities - include what file formats are accepted for PWB
Data (What version of Gerber, drill files, etc.).

 From a survey design point of view, I would also explicitly list several
design and manufacturing software tools in the hope of eliciting better
responses, i.e. (I hope I can name names on Technet- if not, please flame
me offline.  Also note, this is just an example list, there may be
others!)
