My best guess:

1-      What is the minimum bend radius for a 2 layer Flex board with
cover
layer?

10 - 12 times the thickness of the flex.

2-      What is the minimum distance for placement of components form
the
bend line of a flex board?

As some one else stated, you need to keep the components away from bend
or flex areas.  You may want to consider
tie downs or strain relief.
