Fred,

Are you sure your looking at ESD.  I have experienced
problems with lead forming on some TO 220 components.
The problem is: during preform we cause a shock to occur,
travel up the lead and crack the die.  When you apply a little
power to the device-bang!

I certinly would not rule out ESD safe handling practices but
I would check the potential of any shock being sent to the
internal components by the riveting process.  Have a few
components sent out for F/A.  Take some from post riveting
and some from post test.   Also it is a good idea to check the
manufacturer-send out a couple of virgin parts(in the tube).
