Ken,

I would recommend gerber RS 274X.  This form of gerber has the aperture
definitions, paint/scratch definitions, etc. all contained within the
gerber file.  With this format, you would have one RS 274X file per layer.
Every CAM package I know of can read this format.

With regards to drill data, I would suggest either supplying an RS 274X
file with the rest of the data or Excellon format.  Regardless of the
format you supply, the fab house will read it into their CAM software for
panelization, etc.

Independent of the format of the data files, a good readme file describing
what layers each file represents and any other information not contained in
the gerber and drill files.

Finally, an X,Y netlist such as an IPC-D-356 file should also be supplied.
This allows the fab house to compare a netlist extracted from the gerber
and drill data to the supplied netlist to verify that the data is correct.
