Mil-Std-883 has served the industry successfully for many years. It is
fountainhead  of sorts should be on your bookshelf if you are getting in to
reliability testing. There are numerous other conditions that individual
companies and government agencies employ for their internal use based on
their experiences.( e.g. AT&T, NASA etc.) You will likely receive many
responses to your question that will reflect the "scar tissue" from hard won
experiences over the next day or two. As for number of acceptable failures,
that depends alot on who the customer is and the application. However, if
you or I are the customer, I think the answer is zero ; )
