I am looking for information on how to write a contract for doing
circuit card assembly at a Turnkey assembly house.  Specifically, what
are the details on how to setup, write and implement such a contract.

This assembly house will buy parts, assemble, ICT, functionally test all
board assemblies and ship to our company for final assembly.

Some of the key points I'm looking for are how to handle the following:
1. AVL (approved vendor listings)--how does one handle use of the
assembly house suppliers w/o our company approval?
2. Expectations from ICT and functional test as far as yields, fixture
maintenance etc.
3. Warranty and repairs--what is typical for board assembly warranty
i.e. 1 year, 90 days etc.  How are repairs handled and are repairs
treated the same way as new?
4. Record keeping--how long should a board house keep records for
traceability?

There are many other areas as well.  If anyone can offer assistance, I
would be most appreciative.
