Following from the EMPF HelpLine; prepared by IPC-A-610B Master Instructor
Linda Zellars:

"J-STD-610 says:  Nonconforming Process Indicator - Class 1,2,3 for
Blowholes, pinholes, and voids providing the solder connection meets the
requirtements of 4-1.  Table 4-1 states that Class 2 and three have 75% hole
fill.  If you can demonstrate that the solder fills at least 75% of the hole
then you have a process indicator.  If not you have a defect."
