6012 guy here.... The PWB Fabricator configures the coupons. If they don't  know how to, don't use them. 
The IST coupons are not part of the 2221 coupon set and is not part of lot acceptance, unless specifically required by the board user. Refer to IPC-TM-650 2.6.26A for how to conduct IST.
