Some of the options would be CAM350 http://www.downstreamtech.com/cam350.php or Valor https://www.mentor.com/pcb-manufacturing-assembly/
CAM350 is a lower cost solution, very popular with PCB fabs. Valor is a "best you can get" solution, costlier.
Unfortunately I don't have an accurate idea about pricing, I would say 5-10k for CAM350, 30-50k for Valor. This are the installation fees, followed by yearly licence fees.

Sorry some confusion. I am looking for post CAD - using Gerber inputs to generate CAM for pcb manufacture - Gerber 274X inputs and Outputs for Panel up, Drill, rout optimisation, Bare board test pin fixture, DRC , DXF inputs optional.
