You neglected to mention where in the product development/mfring/service stage this failure was occurring; whether it was experienced on an ongoing basis.
You also didn't specify the type of failure or level of stress at which the failure became evident.
All of that information is relevant, when instituting a troubleshooting, failure analysis and long term cure.
You wouldn't want us babbling at length about irrelevant anecdotes (though that doesn't seem to have been an issue this time around).
You also wouldn't want to see a re-occurrence in the next cycle......
