Looks to me like there was some plating resist residue due to incomplete development on the pads that kept them from getting plated copper.
IPC-6012, 3.5.4.2.1, appears relevant although I'm not sure these imperfections fit the T-50 definitions for 'nicks, dents, and pin holes'.
I will say our inspectors on occasion see pads which, qualitatively, look like these. "Qualitatively", since they find sometimes much smaller islands of missing plating than what these pictures show. Pads looking a lot better than those in the pictures get scrapped here.
Let me mention that TechNet has a provision for posting pictures at the IPC site. I do believe there have been some problems with ipc.org this week, however. The CQI conference yesterday revealed the problems arose with Microsoft and are affecting many organizations' websites.

Looks like there was some sort of problem with the etch resist, can't say exactly what that was though. I definitely would be kicking these back to the vendor!
