The magic answer is (as stolen from D. Pauls) IT DEPENDS...  on where in the design spectrum you are working. 
We do teardrop if we are at less than 4/4 tr/sp and at the low end of the spectrum for 2 mil annulus and requiring no breakout (Mil-spec designs). With bigger annular rings, wider traces, and commercial breakout requirements we do not.

snowmans(?)
or any other form of copper "pad to trace" fillet to your conductive layers?
