John,

If I ignore all previous comments on your post and look at this picture I 
see what we used to call an epoxy spot.

It appears that you can see copper through the contaminant that prevented 
the final plate.

We see this in flexible circuits too with no epoxy from prepreg to melt on 
the surface.

Often does occur after lamination.
