Larry,
I've used 20 mil (0.5 mm) pad with a 8 mil (0.2 mm) hole on Class 3 designs for Surgical Medical Instruments.

Subject: [DC] Minimum Via size
Is there a minimum via size for a blind via for Class 3?
I'm working on a design that I will need to use a laser drill of 4 mil.
Thanks,
Larry
